REQUIRED FORMS:
* * signed participant contract
* * copy of birth certificate (new players only)
* * sports physical dated after 1/1/2010 - must be on official
Pop Warner physical form
* * copy of complete 2009-2010 report card must be submitted to league
or Team Parent prior to equipment pick up. Players WILL NOT be issued equipment or be allowed to practice until this requirement is met.
Reminder: everyone that has direct contact with participants
also needs to have a volunteer application/background check completed
and on file (ie: team mom, trainer, etc) - background checks are good for 2 years
*Coaches need to complete both the coach application and the background check
*All other volunteers need only to complete the volunteer/background check
